I could not find this after a long on-line search, so I'm posting what I learned from disassembling the headset.

JVC GY-DV550 camera with KA-300 intercom headset

1 - red - mic hot

2 - white - earphone +

3 - not used

4 - mic shield

5 - black - earphone -

I don't know whether the wiring is the same for other JVC video cameras, but would guess that it is.
